ASCN UK EXPASS Questionnaire

Many of you may be aware of the EXPASS (Exercise and Physical Activity after Stoma Surgery) project. However, even if you are not aware of EXPASS or have limited knowledge of the EXPASS project we very much would appreciate you complete this survey. 

The results of this questionnaire will enable us to bench mark and gain insight into the awareness of the EXPASS best practice recommendations. Your responses will also help us to develop and shape how we can disseminate these national recommendations for your benefit.

There are a total of only 11 questions.  If the questionnaire requires you to complete all 11 questions – it will not take any more than 3 minutes.
